Who can request a birth or death certificate?
For privacy purposes, the following individuals are the only permitted persons that are able to request a birth or death certificate:
  • Registrant - This is the primary person named on the vital record
  • Immediate family member - This includes wife, husband, son, daughter, mother, father, brother, sister, grandmother, grandfather, grandchild, aunt, uncle, niece, nephew and in-law (This does not include cousins).
  • Legal guardian - This includes includes guardian who shows guardian paperwork, foster parent who furnishes custody papers, and step-parent (Please bring your guardianship papers).
  • Legal representative - This includes an attorney acting on behalf of the registrant or family.
