1. Do I have to make an appointment?

Appointments are required for all services (except those involving vital records and the passport program). Please consult the Division of Health Services program sections for more information.

2. Do you accept credit cards?

Effective June 2016, the Department of Public Health accepts payment for selected services using VISA, MasterCard or Discover. In addition, online payments of birth/death records may be ordered with a credit card at an additional cost using a third-party vendor.

3. What do I need for ID?

4. Do you give shots for children, adults, or both? How much do these shots cost?

5. How do I know if I have the flu?

It is very difficult to distinguish the flu from other viral or bacterial causes of respiratory illnesses on the basis of symptoms alone. A test can confirm that an illness is influenza if the patient is tested within the first two to three days after symptoms begin. In addition, a doctor's examination may be needed to determine whether a person has another infection that is a complication of influenza.
