After getting a COVID-19 vaccination, you should receive a vaccination card from the provider that tells the type of vaccine received, the lot number for that vaccination, the date the vaccination was administered and the location of the vaccination. The St. Charles County Immunization Clinic can provide a replacement for those who received a vaccination in Missouri but did not receive this card or have misplaced the card after completing vaccination.
Replacement cards are issued through the Immunization Clinic between 8:15 a.m. and 4:30 p.m. on Wednesdays. The Immunization Clinic is located on the lower level at the Department of Public Health offices — 1650 Boone’s Lick Road in St. Charles. No appointments are necessary, but individuals must have received their vaccination in Missouri, be entered into Missouri’s Show-Me Vax network and present a photo identification (driver’s license, passport or another government issued identification) at the time of pick-up.
Replacement cards cannot be mailed or emailed by the Department of Public Health — they must be picked up in person. Those who obtained vaccinations outside of Missouri should contact the state health department where their vaccination was administered.
