MAPP Workplan Meeting - Chronic Disease

Throughout 2019, community leaders have participated in the MAPP process to improve health in St. Charles County. One focus from this has been to reduce incidences of chronic disease in various populations. This meeting will discuss available resources and develop a strategic plan for directing future activity.
